Server allowing 2 users to open excel file

I have a situation at a customer site that the server is allowing (sometimes)more than one user to open the same excel file, of course this is not good, but I cannot seem to find how it is happening. Any Suggestions?

More than one person can open the file but the second user to open it should have read only access, have you tested if both users can actually save the open document?
 
Within Excel on the Tools menu the user can share the workbook. This enables more than one user to open a workbook in edit mode.
A shared workbook contains the words "[shared]" in the title bar.
