I have granted a user the "Send on behalf of" permission (under delivery options) for one of the companies managers so she can send messages as him.

But sometimes the email message gets sent so that the recipient can see the "sent on behalf of" displayed in the address, but sometimes doesn't. What I am after is so the email looks as though it is from the manager and not sent on his behalf.

Any ideas?
 
Sent on behalf of" and "Send as" are 2 different things.

Grant Send As permissions instead.
 
You can find the "Send as" permission on the ACL of the user account. You can expose the "security" tab by enabling the Advanced view in ADUC.
