It sounds like you haven't set up the database as a system data source name (system DSN). Once that is done, then you must create a database connection to it in FrontPage. On the Tools menu, point to Web Settings, click the Database tab.
If you are trying to do it on a hosted web site, you might need to have your web host set-up the DSN for you.
To insert records into a database you use the "append records" function.
