Send Images through a Form?

I would like to know how I can put a button on a webpage that would allow a visitor to send an image with the form that they're filling out, like when you select a photo in FP2K, you click on the IMAGE icon and put it on the page, I would like visitors to send me images with the form results to my email, without having to send the visitor to their email software to use as an attachment.

The site I'm trying to incorporate this on is:

After the "Type of Band" section on the form, I want a "Send Your CD Cover Photo" button so they could send me the image they want.
