send form values to template

Hi there,
Is it possible to have an html form which the user can fill in, hit the generate/submit button, and have the information entered sent to a word template to populate bookmarks?

So on hitting generate, what the user sees would be MsWord popping up with all the fields filled in.

Any response would be much appreciated, even if to say it's not possible.
