Selecting a word form from a listbox

I would like to create a vba userform which has a listbox a current date label and either a textbox for the user to enter their name or the facility to display the current users name in a label.

The user should be able to select an item from the listbox which correspondse to a word form (all of which will be located in a single subfolder)which it opens and pre-fills form field 1 with the users name.

The basic idea is that users will be able to select a document from the user form and have several bits of information pre-filled. can anybody help
