Select Fields to Show on Report

I have a need to allow my user to select from 56 fields the particular fields in which they would like to print on the form.

If you have any ideas, please let me know.

Built a answer selection form that's based on query or table. Use box checkboxes and pulldowns in you form to help build the report they want to print.
