Searching by primary key and name

How do I create a query to search by medical record number(MEDRECNO), last name(LNAME), and first name(FNAME).

I want to put a button on my switchboard that'll open up a form where you have the option to put in a MEDRECNO, LNAME, or FNAME, click a button to run the search, and open up the main form with the associated record.

Please let me know if I'm not clear and I'll provide more info.

This seems to me to be more of a forms question (forum702). An advanced search of the subject line in that forum using keywords "search criteria" will produce a number of threads.
