Search over three fields - with one question

I need to search with one question to the user over three seperate fields in a query from a table...

We have an order log that has three potential fields the information may be in.

Can someone help ????

not to clear in description but I suggest you look at OR
field 1 = Yes or field2 = yes or field 3 = yes
