Search Form within PDF

I would like to create a simple search form on a page in my PDF. Just a text box and submit button will do. This should pass the search query to the Acrobat Search facility and display the results in the Acrobat search results page.

I'm using Acrobat 6 and this doesn't need to be backwards compatible as it is being kept internally within the company.

Any ideas???
