Script to test if specific ports are open on specific machines from a file

Hello everyone,

I have a list of around 156 servers and the ports that are open on these machines. I know I can telnet the machines on the mentioned port but this will take a lot of time.
The list is in an Excel file. I need a script that can do the check for me; if possible feeding the script the excel file or a csv file and giving me a result like "<HOST> <PORT> open/closed/timeout" in a log file.

The list format is as follows:
Machine Protocol (TCP/UDP) Port
xx.xx.xx.xx TCP 23
