SCREECHING Noise when conferencing calls 2.1.15

I have a 403, 2.1.15 using assorted sets mainly 4424's. Whenever customer goes to conference two external and one internal caller they get this extremely loud screeching noise that everyone hears. I tried it myself from several types of phones within the office and heard it myself. We called different #'s etc... The noise is so loud that you just hang up immediately. This is a new install approx. 1 month ago. Anyone expierence this and/or have suggestions/fix for it.

I have used Conferencing on the IP403 V2.1(15) on two occasions I have also add the same problem.

We discovered on those two occasions that the conferencing chips was faulty.

Have you tried to downgrade the IPO to V2.0(18)

I've heard (and had) enough problem with V2.1(15) that I now recommend ALL IPO's be downgraded to V2.0.18 (the last stable build from avaya) until they manage to get the long promised patch for 2.1 past acceptance testing.
