SC to LC

I have a Pair of HP ProCurve 4000m switches in my plant. They're connected to each other with fiber. In each of the switches is an 100Base-FX Module. The two strands of fiber going between the switches are terminated with SC connectors. I am replacing my switches with a pair of NetGear GSM7248 Switches and AGM731F 1000base-SX fiber modules.

Is it possible to just terminate my fiber with LC connectors or am I going to have to replace the entire cable?

you can reterminate if you have enough slack on the cable....
 
You can just use a SC to LC fiber jumper.

Is correct. They make these in both single and multi mode fiber patch cables.

How can I tell if I have single or multi mode fiber?

Generally by the jacket. Multimode normally has an orange jacket, while single mode uses yellow. The actual fiber strands are also thinner for single mode. The jacket should be marked with the manufacturer, type, and core/cladding diameter.
