I was able to make a replacement cable for this. Just use a regular short straight through network cable. Now the fun part. Using a dreml tool or a file, lop off a corner of the plug on the clip side from front to back at a 45 degree angle. Do this on both top sides (left and right with the business end facing you clip side up, you wont need much and the standard RJ45 will slide right into the special Avaya jack. I plugged it into a 9611G phone and it came up immediately, no need to rewire the plug in a special way.
