Saving an MS Word form template (.dot) as a .doc

I have created a MS Word form template (.dot) file with active X controls, including Combo boxes. When I save the form as a .doc file, the drop down lists disappear. The file works fine as a .dot file with all items in the list displayed, but I need to have the file saved as a .doc (As soon as a user double clicks on the .dot file, Word creates a .doc file, so it will not be possible to use this file as a .dot. Any help on this will certainly be appreciated.
