How can I make a PDF form that can be saved in Adobe Reader after it has been filled out?

For this you will need Acrobat 9 Professional.
There is a menu item "extend properties for Adobe Reader" (or similar, don't have the English version)

It depends on the number of users though. Read the EULA carefully, you might need "Adobe Life Cycle Management" for it.
