save form data in different place?

I need to save form data to a different directory and text file based on a value from a dropdown list on a form. for example If the Physician name Smith is selected from the dropdown list and other form data is filled in I need to save it to d:\formdata\smith\formdate.txt the foldername would corrospond to the physician name from the list any Ideas or pointers on where to start?

If knowledge were power I would be a AAA battery!
 
The start point is to use the FSO to determine if the folder is there or whether to create it for a new entry.

Then check to see if there is a file for that date, if not then create it, else APPEND the new data to the file...
