Save a form as a text (or Word) doc

I have built a form (a questionaire) dynamically and now need to save it as a document so it can be sent to users to print out. I can't quite seem to get it. When I save it as .txt or .rtf it's blank.


I did these manually, but need to do it in code. Any help would be appreciated.
