Satisfying user and pw challenge for CR8/SQL 7 report in web viewer

I am at a loss. It seems so simple.
Here's the HTML:

I click the link, the page comes up nicely asking for my user name and password (DB is predefined). Regardless if I have a user0 and password0 provided in the url, nothing is satisfied.
I can type in the username and pw on the page but again, nothing happens, I get only the same page requesting my username and password again. Yes yes yes... I am sure of the username and password and its permissions, etc... I have typed it enough times so that one would think I have an even odds chance of getting it right atleast once.
The report is a simple one. One table.

Here's something interesting. Here's the challenge:

Server Name : DCS
Database Name : DCS
User Id :
Password :

The database name is correct - DCS. The Server Name is most certainly NOT DCS. I assumed that information like this was imbedded in the report when I added tables to it from an ODBC datasource??
