I've been on that problem for quite a while now.
The customer has an intel motherboard server with Windows 2003 Server and backup exec 11d and a SATA DAT72 Quantum tape drive.
Every 2-3 days, the tape simply disapear from the device manager, so backup exec can't see it anymore.
There is no way to get the drive back without restarting the server. (Not sure about this, but I think it has to be shutdown and restarted)
I tried:
Swapping the tape drive.
Swapping the Promise SATA controller.
Installing different drivers.
The firmware of the drive is currently 07a, and I just noticed 07b is available.
Upgrading the Intel motherboard firmware.
I am clueless...
