Running (Starting) Applications in 9i AS

I've installed Oracle App Server 4.0 Enterprise on NT 4 (sp 6). Everything seems to be ok - all components are running, listners are running (I'm using Oracle HTTP Server (apache)), but all of the default applications are not running. I successfully added a new app, following the documentation, and created a test servlet to run, however the app still shows as down (and I can't access the servlet). The documentation says that you can stop and restart an application, but there doesn't appear to be any "start" functionality. Stopping or Restarting the app throws an error that correctly points out that the app isn't running at all. Any suggestions - am I missing something completely obvious?
