Is there a way to get a rolling average (last column)for each week (for the previous 12 weeks) without including the monthly subtotal (marked by 'Month Name, Year')??

Check out the SUBTOTAL function with the appropriate Function Number. Nested SUBTOTALS are ignored to avoid double-dipping.
