rockwell driver issue

Hi guys, desperate for some help, everytime i boot up i get the message that windows has found new hardware and is locating the drivers for it, it says that its for Rockwell RC56D2, i presume its for the internal modem that i dont use, however, although i have disabled it in device manager it keeps coming up! it caused a system failure and put me into safe mode! i seem to be running ok although now i cant adjust my display settings and this screen is fragmented and HUGE! i have run system scans and have no viruses etc. can anyone help?
 
if you dont use the modem you could open the PC and disconnect it or remove it if its PCI version.
