Rings once and hangs up

I have upgraded my 406 to version 3 and my voicemail pro. Now my analog phones on a 30 port expansion ring once and then the call disconnects. I have also had a situation where calls will ring once on a soft phone and then go to voicemail. The analog extensions do not have voicemail so maybe that is why they disconnect.
I really hope someone can tell me a setting that can fix this and it is not a 3.0 bug.

Is this when you ring one extn or a group?

You are ringing to an analogue phone, with no call appearance no VM, so intheory it should ring to Busy, if engaged or ring if not?

What are you ringing from?

This is for calls from one analog extension to another, which I have verified. Also from a digital phone 4412D+.
I also had today an incoming call on my Phone Manager VoIP softphone. The call came in our PRI, transfered to our third party Auto Attendant and then transfered to me. It rang once and then went away. However instead of going to my voicemail it actually went on hold. It timed out and went back to the auto attendant and then they transfered to our operator and she transfered it to me and it worked. I have had other situations like that so I think it is more than just analog extensions. And part of the V3 upgrade I see one call appearance on each analog extension.
It seams to be intermittent, where a phone that was having problems one day does not the next time I try it, but another will.

At this point I am considering downgrading to 2.1!

Any help with this would be greatly appreciated.
