Ringing on incoming calls

I have a client that says when on a call and a call comes in on another line the ringing is heard on the handset to indicate to the party on the line that the operator needs to place them on hold to retrieve the second call. And that suddenly this feature is not available. How is this done? I thought it was the ringing from another ext that was heard but they say that is not the case.
 
calls will always ring at least once on the phone and if you turn off abbreviated ringing then it will keep ringing. But it doesn't indicate that in the handset, it only rings on the phone.

The joys of customer service! I'd be willing to bet what they hear is the ringing through the speaker being amplified by the handset.
