Ringing back on single line phone after hangup.

I haven't seen this personally happen but my end user tells me when they hang up their phone after an outside call, the call sometimes rings back and no one is there.
They all use single line phones.
It's an ACS R5.
I think it's either user error, not hanging up the phone properly and the system thinks they attempting to transfer, or there is a feature needed on the telephone line like disconnect supervision or something similar.
Anyone have any thoughts ?

You are correct. User error or a hookswitch problem. The handset may be bouncing in the cradle causing a "hookswitch flash". The system then rings back. Sounds like a CSR dept. or the like where they get mad and slam the phone down or just "fire" it into the cradle.
