Some how users that have “Reader” rights are able to make changes (i.e. modify the web page contents) just like if they were administrators. What am I missing?
If the "readers" can change that, they have more than read rights. Keep in mind that if a user is an Administrator on the server hosting sharepoint, full rights will be given to that user in sharepoint aswell, without having to be added as a user in SharePoint.
Have you made sure that those users have not been added with individual rights on lists or libraries etc? The only thing when it comes to content that users should be able to change is a private view of webparts.

Second, here is what’s wired. If I use the SharePoint admin console in Administrative Tools, I can add users from the Domain and give them for instance, read rights to a site, but it will not let that user access the site.
If I try to add users a Domain user from within the Site, it will not let me do it. If I log on the SharePoint server with that user name, then I am able to add the user and then the user can access the site. Does this make any sense?
