Reset Directory/Log Password on M3904 Phone

I've read previous posts about this, but resetting in LD 32 (CPWD l u) did not work. It's an Opt11/CS1000 running Succession 3, and below is the phone IDU info:

M3900 TN: 015 0 00 10
TN ID CODE: M3904

NT CODE: NTMN34BA
COLOR CODE: 70
RLS CODE: 07
SER NUM: A14AD9



I've also flashed the phone, and that doesn't reset the password, so any help would be appreciated.
