Report using calculated fields to calculate other results

I'm trying to figure out how to construct a report that will basically look like a crosstab report but then allow me to use the crosstab results to calculate results. I'll try to make it clearer below.

Col1: physician's name
Col2: Total physician inpatient and outpatient charges for 1998 (January inpatient+January outpatient+February inpatient +February outpatient, etc)
Col3: Total physician inpatient and outpatient charges for 1999
Col3: Total physician inpatient and outpatient charges for 2000
Col4: Total physician inpatient and outpatient charges for 2001
Col5: Total physician inpatient and outpatient charges for 2002

There will be a total line for the entire grouping of physicians.


Calculated fields

Col6: % total for 1998
Col7: % total for 1999
Col8: % total for 2000
Etc.

Col9: Amount of change 1998 to 1999
Col10: Amount of change 1999 to 2000
Col11: Amount of change 2000 to 2001
etc.

Col12: Percent change 1998 to 1999
Col13: Percent change 1999 to 2000
Col14: Percent change 2000 to 2001
