Replace I2004

I need to replace an I2004 phone. Do I need to access the BCM to do this? Can I find out the IP address and Port from the phone like the Call Server?
 
Yes - you need to access the Unified Manager to deregister the phone you're replacing, otherwise you'll be wasting an IP client license. Go under Services, IP Telephony, IP Terminals, Nortel IP Terminals.

IP address will most likely be the LAN1 IP of the BCM. Port is 7000. You may need the registration password also, but if you don't know it you can reset it via the Unified Manager as well.
