In the /etc/hosts.equiv, make sure that you have the correct entries for the host that you are connecting from. Telnet to the remote host, and run `who -Rub`. This will show the host that you connected from. Sometimes in your /etc/hosts file the "localhost" may be resolved by the "remote host" as a different entry other than what you are expecting.
