Remote forward problem 1

I have ext 7111 with a forward button; permissions have been given to forward it off the system. When you choose prompt 3 off the auto attendant, it is supposed to transfer you to ext 7111, which will then forward. It isn't working - if you dial in and choose ext 7111, either by choosing prompt 3 or dialing *T and the extension number, the greeting says "please hold while I transfer you" and the phone rings forever. If you look at the phone, the forward button is green and shows it is going to the correct number. Any thoughts?
 
If you dial 7111 locally do you get the same result?

For the Voice Mail ports, did you unrestrict them, raise their FRL to 3, allow them Trunk to Trunk transfer and RCF?
 
Yes, the same thing happens locally and no, I didn't check the restrictions and FRL on the VM ports - I bet that is at least part of the problem because s/l ports seem to revert to restricted when you rearrange boards. Although I am a little surprised that I don't get wave-off or anything. I'm going to check the restrictions right now...
 
If it happens locally as well, also check for Loop Start Reliable Disconnect set to Yes
