brand new install tried several reboots, next step is reformat the sd card, anyone seen this?

?U is wrong for remote access, just make the destination "voicemail" from the dropdown in ICR, or its ?Anonymous for shortcodes

There is no Vextension configured.
So extension 201 needs V201 in the source number field.
