Relational Database for Network Connectivity survey

I would like to find/create an ACCESS database with multiple tables that could not only create custom printable forms for connectivity "field" surveys, but a database entry form that was intuitive enough to open up the table of the "gaining" connected equipment when selected from a programmed pull-down that is generated when selecting from its rack/desk location.
{example; when entering connection data for a given server,[e.g. EQPMID] you know from your survey that it is connected to a switch in a different rack, the racks, {previously entered from an inventory survey} are available in a pull-down list from the location field that provides RACKID, when the switch's rack location is selected, the next field, the connected equipment, automatically generates a pull-down of all the equipment [also EQPMID] known to exist in that rack (as of survey date).
When selecting the appropriate connected switch, the blade and/or port information from that equipment's record are opened and you merely select which blade/port that your server is connected to. This in turn populates the connected equipment table information in reverse.)

Is there such a template out there or do I have to jump into the deep waters and create this monster myself?

There would have to be tables for every different purpose/type of equipment, (hub, modem, patch panel, router, server, switch, etc.) with at least two "foreign keys" being the location field and the equipment field)
other foreign keys by default would be manuf name and model name/# of equipment or any other control ID necessary for the survey.

Is this descriptive enough? [ponder] :-D
