I have made some changes/additions to some distribution lists.

How can i "refresh" the GAL so the changes are shown in the GAL immediately??

How many Domain Controller's do you have? Depending on which DC you are authenticated to, you might have to force a replication for the changes to be updated on the other DC's. Use Active Directory Sites and Services to force a replication between them.

1 very small site.

1 DC with a seperate Exchange 2003 server - only 18 users.

if you are talking about refreshing in the OUTLOOK clients?
if you are running outllok in Cached mode then it cant be propigated any faster. You have to wait for each client to update ( every 24 hours the client refreshes its GAL against the server) of course you could make the outlook clients run in ( what ever non cashed mode it) and it will be the what is on the server
