Unfortunately this was not a well thought out feature. If the vast majority of your calls are long distance, you can add 91 as a prefix in the line form. However, you won't be able to double click on local calls to call them back this way.
Alternately, you could try to build shortcodes to respond correctly to the dialed number based on a lead 1, number of digits, etc. This can be fairly easy or quite complicated depending on your local calling area.
One nice thing I have noticed in the Toronto area is that analog lines now receive a 1 prefixed on the caller ID if the caller is long distance. This is likely a new DMS feature that should be getting rolled out over time, and the 5ESS may pickup and do also.
