Record jump when inputing data

An employee is working in a database that is split into the front end and back end, while they are entering data (a date, using dashes) the form will "jump" to a different record. This doesn't happen all the time, actually, it didn't happen while I was watching. Does anyone have any ideas?

Keep a close eye on the employee and see if he is also hitting the ctrl key in unison with the shift key when he tries typing the hyphen.

Watch the scroll wheel, especially if using a "special" keyboard with one built in an interesting place.
