I have a client that has had their IP500V2 9.0.300.941 system for a couple of years. Recently their system has decided to do a random reboot. This has happened twice in the past week.
The only common thing that i can find is one person was on the phone & an incoming call came in, then the system rebooted. When I look at the alarms in status I see the following

Abnormal termination DSI exception.
I don't get what DSI exception means or the codes that follow it.
Also i see that both times this has happened I see that the system did a cold start reboot. There was no loss of power to the IT closet.

I would also like to mention that the system is connected to a new battery backup 1500va.

Current config is #1 combo card #2 DS8 #3 Phone2. 3 pots lines to the combo card.

We recently added #2 & #3 because the client moved & expanded

I have tested the Pots lines to see if there was a problem in that area. Lines are in good shape with good current & voltages

Can anyone give me a clue what could possibly be happening here?

Do you have any errors in the configuration? How old is the unit? It's possible that the power supply in it is going bad.
 
there was some errors as the system used to be connected to another one before they moved. I cleared those out.
The system is a few years old & I guess its possible that its a power supply problem. I've seen these go out over time.
So would abnormal termination mean a loss in power?

When the system reboots, the customer tells me that the screens on the phones go white & they don't seem to lose power and it comes back up & works fine for awhile.
 
You could try running monitor set up to 'save to a file' to see what the system is thinking when it crashes. You also may want to make sure the SD card isn't full. You also have a service alarm and a configuration alarm, might not be a bad idea to address those issues too.

Issue with IP Office 500v2 (9.0 SP3) and on Server Edition (9.0 SP3) getting an abnormal termination exception error and the system reboots; issue may also impact 9.0 SP2.
A fatal exception is seen in System Status.
Problem Clarification
IP Office 9.0 SP3 reboots/restarts frequently; issue may affect IP Office Server Edition and IP500v2 control units/expansion modules.

Example of error seen in System Monitor for the :

PRN: .FATAL DSI Exception address=00000014 d=5 pc=f0341760 f040deb8 f0341a70 f05776ec f043dc1c f043ca84 IP 500 V2 9.0.3.0 build 941

Cause
Bug in 9.0 SP2/9.0 SP3.

Solution
Upgrade to IP Office 9.0 SP5 (or higher)

OR

Apply the following IP office critical patch on all 500v2 and SE as per Tech Tip 267

IP Office 9.0 Service Pack 3 - Critical Patch


The critical patch is called CP9_0_3_16_2.zip and contains:

ip500.bin . . . . . . . . . . . . . . . use with 500v2
ip500v2.bin . . . . . . . . . . . . . use with 500v2
ipoffice-9.0.316-2.el6.i586.rpm . . . .with SE
