Re-setting multi-page form fields upon closing

I am a new user of Acrobat 5.0. I have a 2-page form on which I want the fields to be re-set to blank upon closing the form. I can't use "Set Page Action" because the fields on the first page will re-set when the user moves to the second page. I found a menu item "Tools...JavaScript...Set document Action" which I assume is the one to use, but my knowledge of JavaScript is non-existent. Can anyone out there give me a hand regarding the script to use to re-set the entire form?
