I have the problem and the solution.
The problem is caused by two Adobe Photoshop Icon Handler Shell Extensions.
You need to search for the free utility ShellExView on the web, download and run it to see what shell extensions you are running. You will note that two Adobe Photoshop shell extensions are specifically described as "Adobe Photoshop Icons". One is actually called an "icon handler" and the other is simply called "thumbnail".
Use the ShellExView utility to open regedit directly to the registry entry that contains each icon handler, delete the value data (in the right hand window of regedit), then select the key folder on the left (it's a CLSID key folder, so it will be a mouthful, be careful and don't forget to back up the registry first).
Right click each of the keys, go to permissions, and add the user (as opposed to the groups that will be listed) "Administrator". This is the user that you see when you log on in safe mode, but don't see at any other time. Make sure you use the "check name" button to check it.
After that, make sure it's now in the list, then go to advanced and turn off the inherit from parent permission check box. Go back to the main permission window, and now remove all users and groups from permission except the Administrator that you added. This final user should have the full permissions that you can check. Apply and okay everything, then restart to make sure everything is okay. (Don't forget to do this for both icon handler keys).
That should do it.
