Randomly fail to receive or sucessfully send emails

I have seemingly random problems sending & receiving emails from the same people, I have no notification that it has failed. For example I received a reply to an email where I was copied in on the original,but did not receive the original email. My service provider is blueyonder, I use McAfee & Outlook.
