""You are no longer connected to this file"

Has anyone seen this error when trying to save an attached file in Sharepoint?:

"You are no longer connected to this file. Another user may have removed you from it, or saved over it."

The end result is that you lose any changes you've made to the attached file in SharePoint.

Any ideas on how to set permissions, or change the file properties, to get around this error?
