"leave feedback" script for an web portfolio

I'd like to to add a field to my online photography portfolio, allowing people to leave feedback & criticism on the artwork that is on the site. I own my own domain. Can someone help me with where to start to figure out how to do this?

I see that lunarpages.com (my host) has a couple open source bullettin board scripts, a guestbook script, and a chat script
 

1 include a form under your photo providing at least a textarea and a submit button. a hidden field should also uniquely identify the photo against which comments are to be left
2. in the landing page insert the value of the textarea into your database table against the relevant unique picture id.

to stop spam, consider interposing a CATPCHA system either at stage 1 or between 1 and 2

or you might consider a purpose built gallery like menalto's gallery2.
