"In Maintenance" message

I've hooked up Norstar MICS 4.0 that was working at an office a year ago, and has been in storage since then. When I bring it up, the phone (tried on several different ports) flashes for about a minute and then says "in Maintenance". You can't log in or seem to do anything at that point. The light on the MICS stays green. Has someone run into this before?

You were right, we had the fiber card in the first instead of the second slot.
