On occasion, a user will attempt to answer a call and hear the voice greeting as if they dialed our main number. Any ideas on how this could occur?

Steve this is very rare but it may be due to a call forward to the main DID which is the last 4 digits of your organization number because of some strange reason, like a full trunk or programming error.
this are just maybe's, like i said this is very rare.
