Query not recognizing the form!

i have made a simple form that has a text box to put the start date, end date and a listbox to select another criteria needed for a query. I also have a text box that I would like add that would be for nameing the excel workbook that would be created.

The Query is not recognizing the form. Any ideas of how to do this?

Also having trouble making the macro to do the save/export to an excel file.
