Query MSSQL View Data into Word form fields

I am working on a template for our transcriptionists. It is a letterhead template and I would like to be able to type a patient's chart number into the first field, and have the patient's name, address, etc populate into a series of form fields (or similar) in the document.

I created a view on the database the data comes from, but I cannot figure out how to query the view and return the results to the word document.

Any help is appreciated.

You could set up the Word document as a mail merge template using your database as the data source.

That might work.
