How do you load Query Designer? I see the reference in the manuals as Visual Database Tools. Is this a tool I have to load during the installation process?

It is part of Enterprise Manager, but it really is best not to use it as it doesn't allow you to do as much as writing code will let you do. But to get to it, right click on a table, got to Open Table and select Query.
