WHAT I NEED IS A FORM WITH 5 OPTIONS ON IT.

THE USER SELECTS WHATEVER THEY WANT

THIS INFORMATION IS THE CRITERIA FOR A QUERY THEY HIT A SEARCH BUTTON AND A REPORT IS DISPLAYED OR AN ERROR MESSAGE COMES UP/

Create your options form with a list box. Enter your options values. Add the field to your query as your filter as [forms]![formName]![FieldName].

User selects an option from the list box, clicks search to launch the query, query runs with the list selection as the query filter.

Create 5 buttons, when clicked change the caption of the form. Know you now the option and record source of the form.
