Publishing feedback form problem

I have published a 2nd feedback form onto my Intranet (sitting on NT4 server)my problem is that when a user enters information and submits the form the username - password and Domain login box is displayed. This is not the case with the other form - any suggestions!

The Form I produced has an attachment upload element embedded after deleting this the page works as it should but I do need to have this functionality in the Form. The destination folder for the uploaded files has Read Write access allowed but still the LAN security is triggered..
